Record is VG+, side 1 has some light superficial surface marks and hairlines that won't affect play, side 2 has a couple very light hairlines but looks closer to NM. The sleeve is VG, it has a name written in marker on the back near the top edge as well as quite a bit of wear and creasing on the corners and edges and some surface/ring wear.
